In-person Virtually Hybrid option (alternating lessons between a virtual lesson and an in-person lesson)

POLICIES: I have read and understand the studio policies which are attached below. *

I have read the policy document I understand that payment for the upcoming month is due by the 1st of the month and that I will receive an invoice a week before. I understand that I can cancel lessons on the student portal and if they are cancelled 24 hours before the lesson, I will receive a makeup credit. I understand that I can carry no more than 3 makeups at a time and they expire after 60 days

Please list your FIRST, SECOND and THIRD choice of lesson days and times. Please give a range of available times beginning with the time the lesson could start and would need to end. For example: Tuesday evening from 6:30-8:00.
I also work 10-15 hours a week for Sight and Sound Conservatory and am in the middle of launching another business so I will be making some adjustments to my availability at my Home Studio. It will be very tight to fit everyone in-but I will try! Lessons will be scheduled within these timeframes:
Mondays: 4:30-9:00, Tuesdays: 12:00-9pm, Wednesdays: 1:00-9:00, Thursdays: 3:00-9:00, Saturdays: 9:30, 1:00, 3:30
